# Overview

Researchers at SickKids hospital are undertaking a national study to identify quality-indicators for transition to adult care for youths with chronic health conditions. These indicators will be universally applicable, derived by consensus among multiple stakeholders, meaningful to youth and families, and ready for health-care teams and decision-makers to put into practice. The indicators will evaluate and track the transition of success over time. See study summary sheet for more detailed information about goals, purpose, and processes of this study.

# Who can participate? 

Any individuals who have experienced a chronic condition the required the transition from pediatric to adult health care or who have knowledge of or exposure to this transition progress, specifically: 

  1. Youth (age 18 – 24) with a medical diagnosis of a chronic physical, mental, and/or developmental condition and lived/living experience of a transition to adult care.
  2. Parent, guardian, or caregiver of youth (age 18 – 24) with a medical diagnosis of a chronic physical, mental, and/or developmental condition who and lived/living experience of a transition to adult care.
  3. Health-care providers - Any multidisciplinary Canadian healthcare provider involved in transition care delivery.
  4. Health systems leaders/decision makers - Any health-care administrator involved in child health management, leadership, or administrative roles.

All interested Interested participants will be asked to complete a demographic questionnaire that will take about 15 minutes. Only selected individuals will be invited to participate in the study. A total of 160 individuals will be selected. Participants who require assistance will be supported by a member of the study team.

# Study Process

Participation in this study will occur over six months. There are three sessions in this study. Each session will take approximately three to four hours and occur over four to six weeks, beginning in October 2024. See study summary sheet for more detailed information about the process.

All study participation will occur using an online platform.
