Ces orientations et les outils connexes visent à aider les cliniciens de soins primaires et les personnes chargées de la planification et de la prestation de soins primaires virtuels à adopter une approche réfléchie et délibérée dans leur utilisation des modalités de soins virtuels. Ils peuvent également être utiles à ceux qui participent à la coordination de la prestation de soins virtuels, comme les assistants de bureau de médecin.

This guidance and related tools are intended to help primary care clinicians and those planning and delivering virtual primary care to be purposeful and deliberate in their use of virtual care modalities. It may also be useful to those involved in coordinating the delivery of virtual care, such as medical office assistants.

In the spring of 2022, the Alliance did an evidence synthesis and examined exemplars worldwide and found that Integrated Care systems in various jurisdictions, many like the ones that Ontario’s OHTs have been modeled on, are failing to meet their objectives to improve population health outcomes. What we discovered was that many of these more mature systems have discovered they are not realizing their goals by focusing on health systems alone.

This toolkit is designed to support Indigenous organizations with the planning and implementation of community level COVID-19 vaccination clinics, with assistance from mainstream organizations such as local public health units, primary care agencies, and/or hospital settings.
