Our new health equity-focused, online Social Prescribing training modules are designed for health care teams interested in implementing or improving their social prescribing process. These self-directed modules include Getting Buy-in from Interdisciplinary Teams, Co-designing a Process Map, Building an Asset Map, Meeting with Clients, and Using the EMR to Track the Client's Journey, among others. Learn everything from key terms and concepts to practical tips on creating helpful resources tailored to your workplace and working with clients. The modules can be taken individually or as a whole course.

Course duration: Approximately three hours. Once you enroll, you will have a six-month period to complete the course; if you need more time, please email the Social Prescribing Team.

Certificates: A certificate will be generated upon completion of assigned modules.

  • Module 9 — Using Data to Drive Improvement and Program Planning

**ATTENTION all primary care health professionals! This module includes a link to an additional tool, developed in partnership with the Centre for Effective Practice. This 30-minute resource is specifically designed for health professionals in primary care, whether working independently or as part of a small team, to implement social prescribing in their practice.

Technical Requirements: Internet connection, as well as speakers/headphones.
