Diabetes Registered Nurse  - Full-Time Temporary Position (35 hours/week), up to 12 months in duration Interim Replacement (Parental Leave) Bargaining Unit – ONA (Ontario Nurses Association), Region 3 – Local 95

The Diabetes Nurse is a member of the primary care team and is accountable to the Manager, Primary Care and Allied Health Team.

Note: This position will commence in October 2024.

General Responsibilities: 1. Provide a thorough nursing assessment of clients living with diabetes, prediabetes, or at-risk for diabetes, taking into consideration the client’s level of knowledge and readiness, socio-economic and cultural background. 2. Provide individual counseling to clients living with diabetes, prediabetes, or at-risk for diabetes, including their families and caregivers 3. Use group facilitation skills to lead group education and community programs including agency’s Indigenous Diabetes Program 4. Develop client centered health maintenance plans related to diabetes and other chronic diseases. 5. Ensure continuity of care by maintaining up to date medical records, communication with the client’s primary care provider, and other service providers as required 6. Provide regular monitoring of clients within scope of practice, along with other clinical providers

Qualifications: 1. Member in good standing with the College of Nurses of Ontario (CNO). 2. Prior specialization in Diabetes care is required 3. Certified Diabetes Educator (CDE) designation is preferred 4. Clinical experience working in a primary health care setting 5. Knowledge and awareness of issues related to homelessness, immigration, mental health, poverty and discrimination 6. Experience in a community-based setting working with marginalized groups 7. Knowledge of the health care/social service systems and resources available to clients 8. Strong communication, problem solving and conflict resolution skills 9. Ability to manage complex workload and remain flexible 10. Comfortable initiating or adjusting insulin doses is an asset. 11. Fluency in languages other than English is an asset 12. Experience providing care to Indigenous clients is an asset 13. Experience in Indigenous approaches and healing practices is an asset

Compensation: $70,247 to $83,447 per annum, plus 10% in lieu of benefits and holiday pay; 8% in lieu of vacation; and HOOPP (Health Care of Ontario Pension Plan) option.
