Salary-based Primary Care & Interprofessional teams: Introduction to CHC data available at ICES

Presentation made to ICES by Dr. Jennifer Rayner on September 12, 2022. This presentation introduced ICES scientists to the CHC data profile and past projects that have used that data.

This presentation describes the Model of Health and Wellbeing (MHWB) as our roadmap for service delivery, our evaluation framework as the tool we use to demonstrate the MHWB, and the ownership & sharing of CHC data. It describes how we use a common EMR to record standardized team-based data, in the absence of OHIP billing or shadow billing; how that data is stored and accessed for research and qualty improvement; and how it can be used to better evaluate primary care, mental health, and equity.
