# Social Prescribing Online Training

These equity-focused modules were co-created with teams who had implemented social prescribing. They told us what knowledge and resources they wished they had in place when they first got started, and what lessons they've learned along the way. As your team works through these lessons, they'll learn practical tips and tools for creating unique programs tailored to your workplace, clients, and community. You will also learn how to sustain and improve your social prescribing program, track your clients' progress through the social prescribing pathway, and measure health outcomes. Since the training is self-paced and modular, your team will be able to can choose whether to complete the whole course or just the the lessons that are most meaningful to you.
