Online Training for Social Prescribing: Launch Celebration
The Alliance for Healthier Communities created this resource to help Alliance member organizations understand the relationship between client health complexity ("SAMI" score) and panel-size targets.
In this interactive webinar, Dr. Jaky Kueper and Sara Bhatti described the progress made in Phase 1 of the Alliance Practice-Based Learning Network (PBLN) project to develop an AI decision-support tool, described the roadmap for Phase 2, and gathered feedback from participants to inform the Phase 2 co-design process.
This webinar was presented as part of the Alliance's Lunch 'n' Learn series on November 28, 2023. Jo-Ann Vis, a social work researcher at Lakehead University, shared TAKE'N5, a peer support training program developed to support workplace mental health for front-line workers in primary health care settings. Watch the webinar recording below or review the slide deck here.
This webinar was presented by Dr. Sean Kidd and Mariya Bezgrebelna from the Centre for Addiction and Mental Health. Sean and Mariya are scientists whose research explores the health impacts of climate change with a particular focus on how these impacts disproportionately affect people who face barriers, such as housing insecurity or homelessness.
If you have follow-up questions for Sean and Mariya or would like to stay connected, you can reach out to them at ClimateHealth@CAMH.ca.
For this lunch 'n' learn webinar, held on June 22, 2023, we convened a panel of experts in primary health care quality and virtual care delivery from the Alliance for Healthier Communities, Healthcare Excellence Canada, Canada Health Infoway, and Ontario Health. They presented an overview of their organizations' virtual care resources and helped us understand how these resources can be used together when planning and delivering care.
On October 3, 2022, the Ontario COVID-19 Science Advisory Table (OST) released its final brief, a 3-part exploration of how primary care responded to and was affected by the pandemic, and implications for ongoing health system planning and policy.
This Lunch 'n' Learn Webinar was held on November 16, 2022. It features Russ Ford, a health system researcher with a long history of leadership in Ontario's Community Health Services. He was the Executive Director of Stonegate CHC for 10 years and LAMP CHC for 17 years. He is currently pursuing a PhD in social work at McMaster, focusing his research on health system reform.
