This infographic highlights key learnings and best practices to promote vaccine confidence and uptake from our paper, "Using Trusted Relationships and Community-Led Approaches to Promote COVID-19 Vaccine Confidence and Uptake across Ontario" published in the November 2022 issue of Longwoods Healthcare Quarterly. These findings were based on survey results from Alliance member organizations providing vaccinations or participating in our Community Vaccination Promotion - Ontario (CVP-ON) project.

Social Prescribing Guidebook for team-based primary care providers in Ontario

This guidebook has been prepared by the Alliance for Healthier Communities. We represent community-governed primary health care organizations. Toolkit for Action: Integrating Health Equity & Community in Ontario Health Teams

In the spring of 2022, the Alliance did an evidence synthesis and examined exemplars worldwide and found that Integrated Care systems in various jurisdictions, many like the ones that Ontario’s OHTs have been modeled on, are failing to meet their objectives to improve population health outcomes. What we discovered was that many of these more mature systems have discovered they are not realizing their goals by focusing on health systems alone.
