This is a critical moment for our health care system. It presents both a need and an opportunity for transformational change. For over 50 years, the organizations that make up the Alliance for Healthier Communities have been delivering comprehensive primary health care that exemplifies the kind of change we envision for the whole health system, by providing care for people and communities as described in our Model of Health and Wellbeing (MHWB). Alliance members have created connected health neighbourhoods by reaching beyond their doors, creating partnerships, and integrating health and social care.
In this webinar, we will share the Alliance's vision of a system of primary health care based on the MHWB - a vision that, if implemented, has the potential to advance health equity and vastly improve the health and wellbeing of people across Ontario. This evidence-informed vision describes a health neighbourhood model consisting of:
- Interprofessional, collaborative primary health care
- Community governance and engagement
- Population-based approach to health
- A strong data foundation
- A focus on equity and the determinants of health
- Accountability and efficiency
Sarah Hobbs, CEO, and Jennifer Rayner, Director of Research and Policy at the Alliance will present an overview of the Alliance's vision.
